Most people, including physicians, have little awareness of the term Mesotherapy and ever fewer understand what Mesotherapy does. It has recently gained some recognition in the Aesthetic field as an easy treatment for removal of fat deposits and for body sculpting. Yet few can actually define the term Mesotherapy.
This is not surprising, as Mesotherapy was originally developed in France as a technique to treat many disorders — from hearing loss to baldness. The name Mesotherapy is derived from the area treated, that is, the mesoderm, or the middle layer of the skin. Mesotherapy is really an injection technique and originally used to describe an injection into the mesoderm of the skin and was coined to differentiate the technique from the more common terms intramuscular and subcutaneous. It is a nonspecific term, and in most instances, does not refer to fat removal or body sculpting.
Mesotherapy is simply a method of delivering a single drug or a concoction of drugs to a specific layer of the skin. A better term, in my opinion, would have been mesocutaneous. Even allowing that Mesotherapy works, it is really a subcutaneous injection, and not a mesodermal injection. It is a irrefutable fact that fat lies in the subcutaneous layer of the body.
It is also important to realize that there is not even one scientific study on the safety or efficacy of Mesotherapy in the medical literature, and there are no standardized drug regimens used in treating fat. Every practitioner of Mesotherapy just “does his thing” — making any meaningful comparison of results between different doctors impossible. And, to complicate matters further, many practitioners of Mesotherapy are unlicensed individuals.
The various protocols are at the discretion of the individual practitioner, and most include varying quantities of prescription and nonprescription drugs, herbs, vitamins, hormones, and other “unspecified agents.” Most of these agents are manufactured nonsterile, and are intended for oral supplemental use. Some of these agents are considered lipid soluble, and some are not: and both are mixed into the same cocktail.
No one has studied any of these agents for the uses to which they are being put in Mesotherapy, and no one has even studied the interaction of these agents with one another. Using these agents in a manner that they were not intended raises serious safety and efficacy concerns for ethical practitioners.
Currently, Mesotherapy is marketed as a simple alternative to liposculpture: Mesotherapy solutions are injected into localized fat deposits and are said to just miraculously dissolve the fat. It sounds so easy and so simple. Lipostabil (“lipodissolve”) is an active ingredient in most mesotherapy cocktails and is said to dissolve the fat. But Lipostabil (“lipodissolve”) does not actually dissolve the fat; rather, it kills fat cells and releases this oily fat into the tissues. It is the body’s resulting healing process to this injury that removes the byproducts of the process, and the end result is a permanent loss of fat cells in the area of injection.
Lipostabil (“lipodissolve”)is a combination product and is considered a nutritional supplement. It is not intended for injection into the body and is therefore not regulated by the Food and Drug Administration
Practitioners of Mesotherapy have never reported any scientific studies of how well this process actually works, and of course have not reported on the side effects and complications either. Most reports of complications come from Europe, after the unfortunate patient consults another physician. These complications have included skin atrophy, skin necrosis, infection, and irregular pigmentation and irregular skin surfaces.
In summary, Mesotherapy is a non-standardized, nonscientific method which is promoted as a simple alternative to liposculpture. There is absolutely no verifiable information on the safety and efficacy of Mesotherapy in the medical literature. Liposculpture remains as the only proven safe and effective form of fat removal currently available. When considering Mesotherapy, one should adopt an attitude of “caveat emptor.”

The skin does not always fully conform to the body’s new shape following significant weight loss. The skin may lose elasticity and begin to sag. One of the areas of the body most affected by this problem is the thigh area.

This allows for pain injections, intravenous catheters, etc. Bruising and swelling usually subside within a month, strenuous activities are usually possible in six weeks and almost all symptoms are gone in four to six months.

Blue light with a 400 nanometer wavelength by itself is not very useful to doctors for skin treatment. However, by pre-treating the skin with a new class of drugs called photosensitizers, there is a revolutionary new way to treat common skin conditions such a sun damage and acne. It is simple, safe, and efficacious. Not only does it treat the real diseases of aging skin, it produces skin that is clearer and smoother.
The combination of using a sensitizing agents and its activation by a specialized light source is generally called Photodynamic Therapy (PDT). You may recall your dentist activating the restorations of your teeth with a blue light. This is a very similar process called Photoactivation. Currently there are more than 20 different brands of light sources that can be used to perform PDT.
Sun-damaged skin is the primary indication for treatment in my practice. People with fair skins (class I and II skins, for example, English, Irish, Scandinavian and Middle Europeans) almost always have significant sun damage in Southern California. Manifestations of this are scaly, flaky skin, pre-cancers, actinic keratoses, early skin cancers, and many pigment conditions. All of these conditions caused by sun damage, as well as aging, can be effectively treated with ALA-BLU-U. Of great advantage is that one can treat the face, neck, scalp, chest, hands and arms. Because large areas can be treated at one session, it is very cost effective (about $400 – $600 per treatment). Most patients are satisfactorily treated with one or two sessions.
ALA-BLU-U is also effective in disorders of the sebaceous glands, such as common teenage acne, middle-aged female acne, the acne that accompanies rosacea, and the condition of sebaceous hyperplasia (little small, round target lesions with a dimple in the center). Rhinophyma (thick, oily nasal skin, and also a late-stage variant of rosacea), is effectively treated with ALA-BLU-U, too.
The first step in treatment is the application of ALA. The ALA contact time ranges from 30 minutes to 18 hours, and the BLU-U exposure time ranges from 5 to 16 minutes. The variation in exposure and application times varies with the condition being treated, the thickness of the skin, the age of the patient, and the known effects of previous treatment.
The eyes are protected from the BLU-U light by goggles. During exposure time, most patients experience a mild stinging sensation during the treatment, but this is usually quite tolerable. The stinging is often described as like a sunburn.
Following ALA-BLU-U treatment, we advise avoiding the sunlight for 1-2 days. Staying indoors, or wearing a broad hat outdoors will lessen the chance of reactivation of the ALA. This reactivation is not dangerous, but light exposure does seem to set off the stinging sensation all over again.
This stinging (much like the stinging of a sunburn) may last for a few days. This is usually followed by flaky skin shedding as the damaged areas heal. It is perfectly okay to wash your face and put on moisturizers and makeup during the healing process. Two or three Advil and a vitamin C tablet taken regularly every six hours works very well to speed the healing process. The erythema (redness) that follows the treatment usually resolves nicely in two to four weeks.

Laser treatment is an FDA approved non-invasive approach to permanent reduction of facial and body hair. Long term studies in the U.S. and Europe have shown the technology to be effective and safe when done by experienced professionals. It is a convenient way to remove unwanted hair without shaving, waxing, or electrolysis. Women most often treat the lip and chin, under arms, bikini line, and legs. Dark pigment (melanin) in the hair attracts and absorbs the energy of the light emitted by the laser. Hyper-pigmentation is a response of your normal pigment calls to laser stimulation, is basically a dark tan, in a very localized area. Fortunately, it is temporary and will fade over time. If you are tanned however, it is best if you wait four weeks for your tan to fade, to minimize the chances for hyper-pigmentation.

Prominent or protruding ears can be a source of embarrassment and mental anguish for many people, especially children. In the past few decades, thousands of individuals have been physically as well as psychologically helped through a surgical procedure called otoplasty. Otoplasty, sometimes described as “pinning back” the ears, is designed to change their shape and contour. It may be performed on anyone over the age of five or six years old, the age at which ear growth is almost complete. BEFORE SURGERY
Prior to surgery, a medical history is taken in order to evaluate the general health of the patient. A careful examination is also conducted. The type of anesthesia to be used, the procedure, and possible risks and complications are also discussed. Digital photography with imaging is done before surgery. Preoperative instructions may include the elimination of certain drugs that contain aspirin for several weeks before the procedure in order to minimize the possibility of excessive bleeding. Patients will be expected to shampoo their hair with an antiseptic shampoo the night before surgery.
THE PROCEDURE
Otoplasty will be performed in the operating room or at another location, depending upon the patient and plastic surgeons preference. It is done under general anesthesia with the patient asleep. Premedication may be administered to relax the patient.
There are several surgical procedures that are designed to bring the ears closer to the head. A basic procedure involves an incision made at the back of the ear to expose the cartilage. The surgeon may either fold down the cartilage and use sutures to keep the folds together or remove excess cartilage. After reshaping the cartilage, the incisions are closed with small sutures a small suction drain is placed and the ears are covered with bandages.
FOLLOWING SURGERY
Pain connected with the surgery is minimal to moderate and is controlled with oral medication. Bandages are removed within a few days and are replaced with a lighter head dressing. Wearing a tennis headband for a period of time to hold back the ears may also be recommended. Bruising around the area occurs but fades within a few weeks. Some temporary swelling occurs but this condition can be alleviated by keeping the head elevated when reclining. All patients are given an assortment of homeopathic medications designed to substantially reduce swelling and bruising after surgery. Patients should sleep on a soft pillow but not directly on the ears. One to two days after surgery, patients will be encouraged to shampoo their hair. Scars from the incisions fade significantly in time and are, for the most part, inconspicuous because the incisions are made within the creases of the ears. Although patients are usually up and around one or two days after surgery, the decision as to when normal activities can be resumed is determined by your plastic surgeon and is based on the extent of surgery.

Rhinoplasty (or Nose Refinement Surgery) is the most intricate cosmetic surgery performed today. Minor bleeding is common during the first few days following surgery, and you may continue to feel some stuffiness for several weeks. The Surgeon will ask you not to blow your nose for a week or so, while the tissues heal. After 7 to 10 days, all dressings and stitches will be removed.
Most Rhinoplasty patients are up and about within two days, and able to return to school or work a week following surgery. The Surgeon will give you specific guidelines for gradually resuming your normal activities, but generally you should avoid strenuous activities for two to three weeks. You should also avoid hitting or rubbing your nose, or getting it sunburned. Be gentle when washing your face and hair or using cosmetics.
Day by day, your nose will begin to look better and your spirits will improve. Even though it will take months for the final results to emerge, you will be able to enjoy your new look a week or two after surgery.
